We’ve compiled answers to some of the most frequently asked questions about your benefits with Acculevel.
When do my benefits begin?
- 90 days of employment and must be a regular full time employee.
- Medical – Trustmark (Aetna PPO) – Acculevel pays 60% of employee premium and 40% of dependent premium coverages
- Dental – Mutual of Omaha – Acculevel pays 60% of employee premium and 40% of dependent premium coverages
- Vision – Mutual of Omaha – Acculevel pays 60% of employee premium and 40% of dependent premium coverages
- Group Life – Mutual of Omaha – Acculevel pays 100% of group life premium
- Group Accident Death & Dismemberment – Mutual of Omaha – Acculevel pays 100% of AD&D premium
- Voluntary Disability: Short & Long Term – Mutual of Omaha – 100% employee paid
- Voluntary Accidental – Mutual of Omaha – 100% employee paid
- Voluntary Life – Cincinnati Life – 100% employee paid
When am I able to use PTO?
After 6 months of employment employees are eligible for 40 hours of paid time off. Employees are eligible for an additional 40 hours of paid time off after 12 months of full time employment.
When am I eligible for 401k retirement?
- Employees are eligible for 401k retirement after 6 months of employment. Acculevel will match 100% of your salary deferrals up to 3% of your compensation.
- In addition, Acculevel will match 50% of salary deferrals between 3% and 5% of your compensation.
- This safe harbor matching contribution is 100% vested immediately.
- Deductions are taken each pay period.
When do I get paid/what is the payroll schedule?
Pay is biweekly on Thursday.
